Each guest gets their own sheet. For every drink there's space to write down what it is, then four criteria to score from 1 to 5 (half steps are fair game) with a line for notes:

That block repeats so you can score at least five drinks on a sheet — which lines up neatly with how most tastings run: four to six beers or wines, three to five whiskeys.
How to use it
- Print one sheet per guest (two per page works fine if you want to save paper).
- Number the bottles 1–5 and keep the labels hidden if you're going blind.
- Pour small — that's 2–4 oz for beer, about 2 oz for wine, half an ounce for whiskey — and work from light to bold.
- Score each drink on the four criteria and jot a note while it's fresh. "Tastes like Christmas" counts.
- Collect the cards at the end, add up the totals, and crown a winner.
